These two useful forms appear courtesy of Dr David Hillebrandt. They can be used
to help ensure that vital information concerning an
accident and any victims is recorded quickly and
accurately. David suggests printing these forms onto
weatherproof paper.
Note that the patient report form can be made
multilingual simply by annotating the outline of the
human body with arrows etc., although as David points out
one needs to take care to ensure that doctors do not
misinterpret any diagrams as indicating that the victim
has been attacked with bows and arrows!
